Below is a link to our waiver form. As of 06/13/09 we now allow players that are as young as 10 years old. If you are under 18 years of age you must have a waiver form filled out by a parent or legal guardian. Waivers no longer need to be notarized.
If you are under 18 years of age and have not had your parent sign the waiver YOU CAN NOT PLAY.
